LMFT License Renewals for 2008 are due December 31, 2008

To complete the renewal process complete the renewal form and attach to it any required documentation. You only need to fill out the front page on the renewal form if you did not receive an orange audit slip. If you received an orange audit slip, please complete the back of the form and submit materials verifying your continuing education activities (e.g. certificates, transcripts, letters). As soon as your application is processed, you will receive a renewal card stating your next expiration date of December 31, 2010. In order to be considered on time, all renewals must be postmarked on or before December 31, 2008.   If all materials are not received in the Board office postmarked by December 31, 2008, your license will become inactive. All renewal applications postmarked after this date will be assessed a $50.00 late fee and any application assessed a late fee will be required to include documentation of continuing education.  The renewal form can be found on our website at www.lpcboard.org.  If you have any questions regarding the renewal process, please contact the Board office.   


Documentation of Experience Form 

The Documentation of Experience Form is frequently misunderstood by most supervisors and nearly all Counselor Interns. It is thought that this form is only completed when the supervisory period is complete and the intern applies for licensure.  This article intends to clarify some points of confusion.

 There are two purposes for use of this form by the supervisor and the counselor intern:  It is used to apply for licensure and for changing supervisors. In applying for licensure the form is used to validate the experience required by the rules as stipulated in Chapter 7, para. 705, which expounds on the supervision requirements.  Section I is completed by the counselor intern and given to the supervisor for completion of the remaining sections. You should note that by filling out this form and presenting it to the supervisor that you consent to the release of any information, favorable or otherwise, which the supervisor has concerning your employment or your practice. This section also contains the dates of your supervision, your total hours (direct and indirect), the total number of hours-face-to-face with your supervisor, and the total number of supervised experience. It is in this section that a realistic representation of your hours is preferable. The rules indicate the minimum hours needed, but you will most probably have more hours accrued than the minimum. It is better to list those hours than to just list the minimum required by the rules. The supervisor and counselor intern should be in agreement on the number of hours submitted.

 Section II contains the areas of evaluation that are to be completed by the supervisor. The supervisor should be knowledgeable of the counselor intern’s skill level in each of the nine areas listed. This suggests that the supervisor has effectively assisted the counselor intern in developing expertise in methods of the professional mental health counseling practice and in developing self-appraisal and professional developmental strategies.  The supervisor then recommends or declines the counselor intern licensure and both parties sign the document.

This form is also used when changing supervisors. When a change is implemented, the outgoing supervisor must complete the form with the counselor intern and send it to the LPC board. The20licensing and supervision committee will review the form and approve or decline the change based on the accuracy and completeness of the form. If extreme conditions exist, such as the unfortunate death of a supervisor, the counselor is requested to submit the form with as much information as possible regarding any extreme conditions and the committee will make recommendations to the board for acceptance of the hours presented. For each document submitted with such conditions, they will be reviewed on a case by case basis.

Remember to submit your documentation in a timely manner. The LPC Board encourages all supervisors to review the information contained in this evaluation with the supervisee prior to submitting it to the Board.

Notify the Board in writing of your change of address. Be sure to let us know what address you wish to use for correspondence and what address you wish to use for the website. There is a rule which gives you 30 days to notify the Board of any and all changes in name, address, and phone number. Failure to comply with this rule will result in a $50.00 fine.
