Dear Prospective Applicant:
The Grandfathering Application for Title Licensure as a Licensed Marriage and Family Therapist is on the website listed under FORMS. The application fee is not refundable. The grandfathering requirements and board rules needed to begin the grandfathering period and the mental health counselor licensing act as well as the grandfathering requirements are on the website. This grandfathering licensure has begun under emergency rules and these rules are subject to change until adopted by the board as "final" rules. General and specific instructions on how to apply are included in the application.

Needed to Apply
If you decide that you meet the
requirements and elect to apply, you are required to submit to the Board the
following documents:
1. completed application form
notarized;
2. recent 2 in. x 3 in.
photograph signed on the back;
3. statement of practice;
(sample and directions on website)
4. nonrefundable application fee
of $200.00 in the form of a money order, cashiers check or bank draft. (PERSONAL CHECKS NOT ACCEPTED AND FEES
ARE NONREFUNDABLE).
Type or print legibly and
complete all sections of the application. You may need to ask for transcripts or
send verification forms directly to licensing boards or AAMFT depending the
Option you are applying under. Specify any name changes from your name appearing
on the transcripts or other information.
Mail all correspondence to the
Board office at the above address. It is your responsibility to see that all
required documents reach the Board office. Once the Board has received all of
the required documentation, the Board will determine your eligibility for
licensure.
The Board meets the third Friday
of most months. Be sure to
check our website for meeting dates.
Materials for licensure must be in the Board office the Friday before
the meeting.
