Dear Prospective Supervisor:

The Board of Examiners for Licensed Professional Counselors is implementing a procedure whereby the requisite information for documenting your credentials as a prospective supervisor of a Counselor Intern may be submitted to the Board office prior to your being reviewed and approved for supervision of a particular intern. The following information must be on file in the Board office:

(1) a current copy of your Declaration of Practices and Procedures Statement;

(2) official verification of your supervisor training (a transcript or a certificate of training); At least one graduate-level academic course. Course must include at least 45 clock hours of supervision training. Professional training program must be minimum of 25 direct clock hours with trainers that meet presentation standards established by the Board.

(3) a letter from a colleague or supervisor verifying that you have been in practice for 5 years. Two of the five years must be post licensing experience. (Letter must state where, when, how long, type of practice, and relationship of writer to supervisor.);

(4) a list of all interns you are presently supervising whether under the old rules (prior to Jan. 1, 1993) or the new rules; (Include intern's complete name, address, whether registered as a Counselor Intern or under the old supervision rules).

(5) areas of expertise with validation (transcript, certificate, letter of verification from supervisor, etc.)

The procedure is ONLY designed to allow you to submit all your credentials at one time. This procedure will circumvent your having to submit your documentation with each individual intern.

Remember: No supervisor may supervise more than a TOTAL of 10 Counselor Interns inclusive of both old and new rules at any one time.

Please include a cover letter explaining that you are submitting these materials for the purpose of documenting supervision credentials. Mail all correspondence to the Board office at the above address. It is your responsibility to see that all required documents reach the Board office.
