Dear Licensed Professional Counselor:

This letter serves as a reminder to you to renew your license to practice mental health counseling. The deadline for renewal is June 30, 2005. To ensure that there will be no delay in the renewal of your license, please submit the following:

1. A completed and signed Renewal Application Form (front only unless an audit slip is attached or if your application is late).

2. The $150.00 Renewal Fee (payable to the LPC Board of Examiners; personal checks are accepted).

It is important that you forward the two above items together to the Board office. Applications containing only partial information cannot be processed for renewal. Renewal applications must be postmarked no later than June 30, 2005. Licenses for individuals who fail to meet this deadline will become inactive, and the counselor will be prohibited from practicing Mental Health Counseling until the license is renewed.. All renewals postmarked after June 30th will be assessed a $50.00 late fee.

If during the past two years you have had a name change or if you need a replacement license because yours has been lost, damaged, or destroyed, please check the appropriate line on the renewal application and include the appropriate fee ($25.00 for a duplicate license; $50.00 for name change and new license.) To change your name on your license and in our records, please also include documentation of the name change (i.e., copy of marriage certificate or driver's license) and send the old license back to the Board.

Materials verifying your continuing education activities (e.g., certificates, transcripts, letters) should only be sent to the Board office if you received an audit slip in your renewal packet asking for documentation or if you are late renewing your license. In those two cases, you should fill out the back of the renewal form and attach the documentation of continuing education.

You are no longer required to submit a copy of your Declaration of Practice and Procedures Statement when you renew your license. However, if you make changes to your Declaration Statement at any time, you must send a copy of the new Statement to the Board so that the current version is always on file.

As soon as your application is processed, you will receive a renewal card stating your next expiration date of June 30, 2007. If you have any questions regarding the renewal process, please contact the Board office.
